It can affect both adults and children, causing acute liver inflammation.<\/span><\/p>\n<p><span data-contrast=\"none\">Once acute infection has resolved, it is estimated that up to 10% of those infected in adulthood, and up to 90% of those infected in infancy, will go on to develop chronic infection.<\/span><\/p>\n<p><span data-contrast=\"none\">Chronic hepatitis B (CHB) can lead to cirrhosis (scarring of the liver) and liver failure and\/or <\/span><span data-contrast=\"none\">hepatocellular carcinoma<\/span><span data-contrast=\"none\"> (liver cancer<\/span><span data-contrast=\"none\">). Liver cancer is the second leading cause of cancer deaths globally. There is no cure for CHB, although there are some antiviral medications that can reduce potential liver damage.<\/span><\/p>\n<h2>What to look for<\/h2>\n<p><span data-contrast=\"none\">The incubation period for hepatitis B ranges from 45 to 180\u202fdays. The infectious period starts from several weeks before the onset of symptoms <\/span><span data-contrast=\"none\">(if present)<\/span><span data-contrast=\"none\">, and usually lasts approximately 4 to 5\u202fmonths. Those who develop CHB\u202fusually remain infectious for life.<\/span><\/p>\n<p><span data-contrast=\"none\">Initial hepatitis B infection is often asymptomatic, meaning a person may be unaware that they are carrying the infection. In those who develop signs of acute hepatitis B infection, symptoms may include:<\/span><\/p>\n<ul>\n<li><span data-contrast=\"none\">loss of appetite<\/span><span class=\"NormalTextRun SCXW127885883 BCX0\">on surfaces for at least 7 days<\/span><span class=\"NormalTextRun SCXW127885883 BCX0\"> and <\/span><span class=\"NormalTextRun SCXW127885883 BCX0\">rema<\/span><span class=\"NormalTextRun SCXW127885883 BCX0\">ins<\/span><span class=\"NormalTextRun SCXW127885883 BCX0\"> capable of causing infection<\/span><span class=\"NormalTextRun SCXW127885883 BCX0\"> during this time<\/span><span class=\"NormalTextRun SCXW127885883 BCX0\">.<\/span><\/p>\n<h2>Epidemiology<\/h2>\n<p><span class=\"TextRun SCXW35296000 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"none\"><span class=\"NormalTextRun SCXW35296000 BCX0\">Humans are the only known <\/span><span class=\"NormalTextRun SCXW35296000 BCX0\">host for HBV. <\/span><span class=\"NormalTextRun SCXW35296000 BCX0\">Around <\/span><span class=\"NormalTextRun SCXW35296000 BCX0\">296 million people globally <\/span><span class=\"NormalTextRun SCXW35296000 BCX0\">are <\/span><span class=\"NormalTextRun SCXW35296000 BCX0\">living<\/span><span class=\"NormalTextRun SCXW35296000 BCX0\"> with <\/span><\/span><span class=\"TrackChangeTextInsertion TrackedChange SCXW35296000 BCX0\"><span class=\"TextRun SCXW35296000 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"none\"><span class=\"NormalTextRun SCXW35296000 BCX0\">CHB<\/span><\/span><\/span><span Australia, it is estimated that there are over 200,000 people living with diagnosed CHB, representing around three quarters of the total number of cases (with one quarter remaining undiagnosed). Over 90% of new CHB cases in Australia are contracted overseas.<\/span><\/p>\n<p><span data-contrast=\"none\">Aboriginal and Torres Strait Islander peoples, people who inject drugs, men who have sex with men (MSM), and those born in areas endemic with HBV carry the highest burden of disease in Australia.<\/span><\/p>\n<h2><span class=\"TextRun MacChromeBold SCXW97015892 BCX0\" lang=\"EN-AU\" data-contrast=\"none\">via the <\/span><a href=\"https:\/\/www.health.gov.au\/resources\/publications\/national-immunisation-program-schedule?language=en\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">National Immunisation Program (NIP)<\/span><\/a>;<span data-contrast=\"none\">\u00a0<\/span><span data-contrast=\"none\">a single dose <\/span><span This is complicated by this patient cohort having a diminished immune response to hepatitis B vaccination. Whilst the cause of this suboptimal response is not completely understood, it has been established that the earlier a patient is vaccinated in the disease progression, the better the response and more long-term protection they will have.<\/span><\/p>\n<p><span data-contrast=\"none\">MVEC recommends using the combination hepatitis A and B vaccine, Twinrix (720\/20) in this patient group (see Table 3). There is <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20630640\" target=\"_blank\" rel=\"noopener\">evidence to suggest<\/a> that using this vaccine can enhance seroconversion. Each dose is 1.0 mL given intramuscularly.<\/span><\/p>\n<\/div><\/div><\/div><\/li><li class=\"mpc-accordion__item \"><div class=\"mpc-accordion-item__heading mpc-transition mpc-typography--default\"><div data-id=\"mpc_icon-566a3c64d3853e7\" class=\"mpc-icon mpc-init mpc-transition mpc-effect-fade mpc-icon-hover \"><div class=\"mpc-icon-wrap\"><i class=\"mpc-icon-part mpc-regular mpc-transition  eti eti_arrow_right-down\"><\/i><i class=\"mpc-icon-part mpc-hover mpc-transition  eti eti_arrow_left-up\"><\/i><\/div><\/div><h3>Hepatitis B non-responders<\/h3><\/div><div class=\"mpc-accordion-item__content mpc-container\" ><div class=\"mpc-accordion-item__wrapper mpc-typography--default\"><div class=\"uncode_text_column\" ><p><span data-contrast=\"none\">A non-responder to hepatitis B vaccination is any person with a documented age-appropriate vaccine history, who has an anti-HBs level of &lt;10 m lU\/mL 4 to 8 weeks following the final vaccine dose.<\/span><\/p>\n<p><span data-contrast=\"none\">Recommendations for how to promote seroconversion in this population vary with different studies. MVEC recommends the following immunisation pathway for hepatitis B non-responders: <a href=\"https:\/\/mvec.mcri.edu.au\/wp-content\/uploads\/2014\/07\/MVEC-hepatitis-B-non-responders_-December-2024.pdf\" target=\"_blank\" rel=\"noopener\">MVEC pathway for hepatitis B non-responders \u2265 12-months of age (December 2024)<\/a>.<\/span><\/p>\n<p><span data-contrast=\"none\">Persistent non-responders should be informed of their immune status and advised to minimise the risk of exposure.<\/span><\/p>\n<p><span data-contrast=\"none\">Hepatitis B immunoglobulin (HBIG) may be given to non-immune persons within 72 hours of exposure to prevent infection.
