Vaccine errors, relating to the way a vaccine is stored, prepared or administered, are a type of adverse event following immunisation (AEFI). Diluent-related errors occur when only the diluent of a vaccine requiring reconstitution is administered, or the incorrect diluent is used to reconstitute a vaccine manufactured in a powered or pellet form.<\/span><\/p>\n

In general, where only a diluent has been administered, the vaccine should be repeated ideally on the same day or as soon as possible.<\/span>\u00a0<\/span><\/p>\n

The diluent supplied with\u00a0<\/span>Infanrix\u00a0hexa<\/span><\/b>\u00a0contains\u00a0the\u00a0DTPa-hepB-IPV (b\u1ea1ch h\u1ea7u<\/a>–u\u1ed1n v\u00e1n<\/a>-acellular ho g\u00e0<\/a>, hepatitis B<\/a>, inactivated poliovirus<\/a>) components as liquid in a syringe. If only the Infanrix\u00a0hexa\u00a0diluent is administered, the vaccinee will not receive the Hib<\/a> (haemophilus\u00a0influenzae type B)\u00a0component\u00a0of the vaccine. In this instance, a dose of\u00a0ActHIB\u00a0should be given either at the same time or as soon as the error is\u00a0realised. This will provide protection against Hib without increasing the chance of vaccine reactions due to the\u00a0additional\u00a0dose of other antigens if Infanrix\u00a0hexa\u00a0were repeated.<\/span>\u00a0<\/span><\/p>\n

If\u00a0doses are packaged in multipacks,\u00a0there is an added complication\u00a0of\u00a0identifying\u00a0which individuals\u00a0have been\u00a0impacted\u00a0by a diluent error.\u00a0In the case of\u00a0a discrepancy in diluent\u2013antigen count\u00a0in a multipack, it is difficult to narrow down which individuals are\u00a0impacted.<\/span><\/p>\n

Any adverse event following immunisation (AEFI), including diluent errors, should be reported to the\u00a0<\/span>adverse event reporting service<\/span><\/a>\u00a0in your\u00a0jurisdiction.<\/span><\/p>\n
