Most types of epilepsy do not have a high risk of VPS. However, there are some genetic epilepsies with a strong association with VPS. These include Dravet syndrome and genetic epilepsy with febrile seizures plus (GEFS+).<\/span><span data-ccp-props=\"{&quot;134233117&quot;:false,&quot;134233118&quot;:false,&quot;201341983&quot;:0,&quot;335551550&quot;:1,&quot;335551620&quot;:1,&quot;335559738&quot;:270,&quot;335559739&quot;:0,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<h3>Dravet syndrome<\/h3>\n<p><span class=\"TextRun SCXW239798839 BCX0\" lang=\"AR\" xml:lang=\"AR\" data-contrast=\"none\"><span class=\"NormalTextRun SCXW239798839 BCX0\">Dravet syndrome is a rare and severe form of infantile onset epilepsy with a prevalence of 1 in 15,500 \u2013 40,000 children. It is<\/span><span class=\"NormalTextRun SCXW239798839 BCX0\"> caused by <\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">a genetic mutation, usually a new\/non inherited (de novo) change in the sodium channel gene (SCN1A). Children with Dravet syndrome classically present <\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">with prolonged febrile seizures<\/span><span class=\"NormalTextRun SCXW239798839 BCX0\"> in the first year of life<\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">. Seizures can be hemiclonic (one-sided) or generalised tonic-clonic<\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">. They<\/span> <span class=\"NormalTextRun SCXW239798839 BCX0\">C\u00f3 th\u1ec3<\/span><span class=\"NormalTextRun SCXW239798839 BCX0\"> be sustained (lasting an extended period of time) such as in s<\/span><span class=\"NormalTextRun CommentStart SCXW239798839 BCX0\">tatus epilepticus (SE)<\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">. <\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">Following <\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">c\u00e1c <\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">first seizure, children<\/span><span class=\"NormalTextRun SCXW239798839 BCX0\"> with Dravet syndrome<\/span><span class=\"NormalTextRun SCXW239798839 BCX0\"> C\u00f3 th\u1ec3 <\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">develop refractory (uncontrolled) afebrile seizures, particularly myoclonic seizures, and a plateau or regression in their development. <\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">In approximately 30% of children with Dravet <\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">syndrome, t<\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">he first seizure is vaccine-proximate (often occurring following the 4 or 6 month vaccines)<\/span><span class=\"NormalTextRun SCXW239798839 BCX0\">. <\/span><\/span><span class=\"EOP SCXW239798839 BCX0\" data-ccp-props=\"{&quot;134233117&quot;:false,&quot;134233118&quot;:false,&quot;201341983&quot;:0,&quot;335551550&quot;:1,&quot;335551620&quot;:1,&quot;335559738&quot;:270,&quot;335559739&quot;:0,&quot;335559740&quot;:259}\">\u00a0<\/span><strong>VPS occurring in infants, particularly if they are prolonged or classed as SE, should raise suspicion of Dravet syndrome. <\/strong><strong>\u00a0<\/strong><\/p>\n<p><span class=\"TextRun SCXW55283732 BCX0\" lang=\"AR\" xml:lang=\"AR\" data-contrast=\"none\"><span class=\"NormalTextRun SCXW55283732 BCX0\">Immunisation can <\/span><span class=\"NormalTextRun SCXW55283732 BCX0\">cause<\/span><span class=\"NormalTextRun SCXW55283732 BCX0\"> an earlier age of seizure onset in children with Dravet syndrome. However, there is no difference in developmental or seizure outcomes in children who had a first seizure proximate to immunisation compared to children <\/span><span class=\"NormalTextRun SCXW55283732 BCX0\">whose first seizure was<\/span> <span class=\"NormalTextRun SCXW55283732 BCX0\">unrelated to immunisation.<\/span><\/span><span class=\"EOP SCXW55283732 BCX0\" data-ccp-props=\"{&quot;134233117&quot;:false,&quot;134233118&quot;:false,&quot;201341983&quot;:0,&quot;335551550&quot;:1,&quot;335551620&quot;:1,&quot;335559738&quot;:270,&quot;335559739&quot;:0,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<h3><strong>Genetic epilepsy with febrile seizures plus (GEFS+)<\/strong><\/h3>\n<p><span data-contrast=\"none\">Genetic epilepsy with febrile seizures plus (GEFS+) describes a spectrum of seizure disorders of varying severity. A diagnosis of GEFS+ is usually made in individuals who are experiencing febrile seizures beyond the usual age range of up to 6 years and who have family members across different generations with a history of different types of epilepsy involving febrile seizures and\/or recurrent afebrile seizures.<\/span><span data-ccp-props=\"{&quot;134233117&quot;:false,&quot;134233118&quot;:false,&quot;201341983&quot;:0,&quot;335551550&quot;:1,&quot;335551620&quot;:1,&quot;335559738&quot;:270,&quot;335559739&quot;:0,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">Individuals with GEFS+ may experience VPS.<\/span><span data-ccp-props=\"{&quot;134233117&quot;:false,&quot;134233118&quot;:false,&quot;201341983&quot;:0,&quot;335551550&quot;:1,&quot;335551620&quot;:1,&quot;335559738&quot;:270,&quot;335559739&quot;:0,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<h2>Immunisation<\/h2>\n<p><span class=\"TextRun SCXW123443839 BCX0\" lang=\"AR\" xml:lang=\"AR\" data-contrast=\"none\"><span class=\"NormalTextRun SCXW123443839 BCX0\">Whilst vaccination is generally recommended in individuals with Dravet syndrome and other genetic epilepsies, there is <\/span><span class=\"NormalTextRun SCXW123443839 BCX0\">a well-reported association with febrile seizures\/SE following vaccination.<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\">\u00a0Seizures<\/span> <span class=\"NormalTextRun SCXW123443839 BCX0\">mo<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\">st<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\"> common<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\">ly<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\"> occur<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\"> within the first 48 hours following <\/span><span class=\"NormalTextRun SCXW123443839 BCX0\">inactivated vaccine<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\">S<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\"> (e.g., influenza vaccines, COVID-19 vaccines, Infanrix hexa) and 5-10 days <\/span><span class=\"NormalTextRun SCXW123443839 BCX0\">ti\u1ebfp theo<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\"> live-attenuated vaccines (such as the measles-mumps-rubella<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\">, <\/span><span class=\"NormalTextRun CommentStart SCXW123443839 BCX0\">MMR<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\">,<\/span><span class=\"NormalTextRun SCXW123443839 BCX0\"> vaccine given at 12 months of age).<\/span><\/span><span class=\"EOP SCXW123443839 BCX0\" data-ccp-props=\"{&quot;134233117&quot;:false,&quot;134233118&quot;:false,&quot;201341983&quot;:0,&quot;335551550&quot;:1,&quot;335551620&quot;:1,&quot;335559738&quot;:270,&quot;335559739&quot;:0,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<h3>Recommendations and management<\/h3>\n<p><span data-contrast=\"none\">Children with Dravet syndrome or GEFS+ and other children at risk of vaccine-proximate seizures should be referred to a specialist immunisation clinic for assessment prior to vaccination.<\/span><span data-ccp-props=\"{&quot;134233117&quot;:false,&quot;134233118&quot;:false,&quot;201341983&quot;:0,&quot;335551550&quot;:1,&quot;335551620&quot;:1,&quot;335559738&quot;:270,&quot;335559739&quot;:0,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">Vaccination requires a comprehensive discussion between the patient\u2019s family and the treating medical teams. Usually, expert advice from both immunisation specialists and neurologists is required and a clear plan is devised to ensure that vaccines can be administered under careful medical supervision (refer to protocol in resources). Vaccine administration for these groups includes admission for immunisation under observation and sometimes the prophylactic administration of paracetamol and benzodiazepines. Whilst there is no published data evaluating prophylactic measures to prevent seizures in children with Dravet syndrome undergoing immunisations, there is anecdotal evidence that this can reduce the rate of immunisation-related seizures and SE.<\/span><span data-ccp-props=\"{&quot;134233117&quot;:false,&quot;134233118&quot;:false,&quot;201341983&quot;:0,&quot;335551550&quot;:1,&quot;335551620&quot;:1,&quot;335559738&quot;:270,&quot;335559739&quot;:0,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">Individuals with stable epilepsy, who have not had a previous VPS and whose type of epilepsy is not strongly associated with VPS, can generally be vaccinated safely in the community.
