These conditions result in increased susceptibility to infections,\u00a0including vaccine<\/span><span data-contrast=\"auto\">\u3002<\/span><span data-contrast=\"auto\">preventable diseases<\/span><span data-contrast=\"auto\">, as well as a higher risk of autoimmunity and autoinflammatory disease. This is because they are unlikely to mount a meaningful immune response, and they are considered protected against most vaccine\u2011preventable infections through their ongoing immunoglobulin therapy. Although inactivated vaccines are safe in this population, they are\u00a0<\/span><span class=\"NormalTextRun SCXW161283233 BCX0\">generally not<\/span><span class=\"NormalTextRun SCXW161283233 BCX0\">\u00a0administered in routine clinical practice due to their limited effectiveness.<\/span><\/span><\/p>\n<p><span class=\"TrackedChange SCXW133823080 BCX0\"><span class=\"TextRun SCXW133823080 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW133823080 BCX0\">This<\/span><\/span><\/span><span class=\"TextRun SCXW133823080 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW133823080 BCX0\">\u00a0approach<\/span><\/span><span class=\"TrackedChange SCXW133823080 BCX0\"><span class=\"TextRun SCXW133823080 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW133823080 BCX0\">,<\/span><\/span><\/span><span class=\"TextRun SCXW133823080 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW133823080 BCX0\">\u00a0differ<\/span><\/span><span class=\"TrackedChange SCXW133823080 BCX0\"><span class=\"TextRun SCXW133823080 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW133823080 BCX0\">\u8033\u9f3b\u5589\u79d1<\/span><\/span><\/span><span class=\"TextRun SCXW133823080 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW133823080 BCX0\"> from the recommendations in the Australian Immunisation Handbook, <\/span><\/span><span class=\"TextRun SCXW133823080 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW133823080 BCX0\">reflects\u00a0<\/span><span class=\"NormalTextRun SCXW133823080 BCX0\">current\u00a0<\/span><span class=\"NormalTextRun SCXW133823080 BCX0\">standard practice among paediatric immunologists in Australia and internationally.<\/span><\/span><\/p>\n<p><span class=\"NormalTextRun SCXW27208289 BCX0\">\u4e00\u4e2a\u00a0<\/span><span class=\"NormalTextRun SCXW27208289 BCX0\">exception to this\u00a0<\/span><span class=\"NormalTextRun SCXW27208289 BCX0\">broad\u00a0<\/span><span class=\"NormalTextRun SCXW27208289 BCX0\">recommendation is the inactivated\u00a0<\/span><span class=\"NormalTextRun SCXW27208289 BCX0\">injected\u00a0<\/span><span class=\"NormalTextRun CommentStart SCXW27208289 BCX0\"><a href=\"https:\/\/mvec.mcri.edu.au\/references\/influenza-vaccine-recommendations\/\"><div id=\"space\"><\/div><\/a>\u00a0<\/span><span class=\"NormalTextRun SCXW27208289 BCX0\">vaccine, which is routinely recommended on an annual basis for people receiving <\/span><span class=\"NormalTextRun SCXW27208289 BCX0\">IRT<\/span><span class=\"NormalTextRun SCXW27208289 BCX0\">. This is because people receiving IRT may still mount a T-cell response that confers partial protection or reduces disease severity, and because immunoglobulin products are unlikely to contain protective antibodies against current circulating influenza strains due to the lag between plasma donation and distribution of immunoglobulin products (7\u201312 months) . <span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\">For similar reasons, people receiving IRT are also recommended to receive a primary series of <\/span><span class=\"NormalTextRun CommentStart SCXW45943948 BCX0\"><a href=\"https:\/\/mvec.mcri.edu.au\/references\/covid-19\/\">\u65b0\u51a0\u80ba\u708e<\/a> vaccination\u00a0<\/span><span class=\"NormalTextRun SCXW45943948 BCX0\"><div id=\"space\"><\/div>\u00a0<\/span><span class=\"NormalTextRun SCXW45943948 BCX0\"><div id=\"space\"><\/div><\/span><span class=\"NormalTextRun SCXW45943948 BCX0\">\u00a0\u52a0\u5f3a\u5242\u91cf<\/span><\/span><span class=\"TrackedChange SCXW45943948 BCX0\"><span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\">, and adults 18 years and older are recommended to receive\u00a0<\/span><\/span><\/span><span class=\"TrackedChange SCXW45943948 BCX0\"><span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\">2 doses of\u00a0<\/span><\/span><\/span><span class=\"TrackedChange SCXW45943948 BCX0\"><span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\">inactiv<\/span><\/span><\/span><span class=\"TrackedChange SCXW45943948 BCX0\"><span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\">at<\/span><\/span><\/span><span class=\"TrackedChange SCXW45943948 BCX0\"><span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\">\u7f16\u8f91 <a href=\"https:\/\/mvec.mcri.edu.au\/references\/zoster\/\">herpes zoste<\/a><\/span><\/span><\/span><span class=\"TrackedChange SCXW45943948 BCX0\"><span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\"><a href=\"https:\/\/mvec.mcri.edu.au\/references\/zoster\/\">r<\/a>\u00a0<\/span><\/span><\/span><span class=\"TrackedChange SCXW45943948 BCX0\"><span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\">vaccine at the recommend<\/span><\/span><\/span><span class=\"TrackedChange SCXW45943948 BCX0\"><span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\">\u7f16\u8f91<\/span><\/span><\/span><span class=\"TrackedChange SCXW45943948 BCX0\"><span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\">\u00a0time interval<\/span><\/span><\/span><span class=\"TextRun SCXW45943948 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW45943948 BCX0\">.<\/span><\/span><\/span><\/p>\n<h2>\u7981\u5fcc\u75c7<\/h2>\n<p>Live\u2011attenuated vaccines are contraindicated in individuals with IEI and should not be given (even when receiving IRT) because of concerns regarding both safety and efficacy.<\/p>\n<h2>Other considerations<\/h2>\n<p><span data-contrast=\"auto\">In some cases, IRT is administered on a temporary basis, such as prior to definitive treatment of an IEI with <a href=\"https:\/\/mvec.mcri.edu.au\/references\/cancer-immunisation-guideline-chemotherapy-and-post-haematopoietic-stem-cell-transplant_2\/\">haematopoietic stem cell transplantation (HSCT)<\/a>, or in conditions where therapy is provided for a defined period before immune function is reassessed. Immunisation status should be reviewed following cessation of treatment. In most cases, routine vaccinations can be recommenced according to the standard immunisation schedule.<\/span><\/p>\n<p><span class=\"TextRun SCXW235922310 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW235922310 BCX0\">For family members\/carers of people with IEI receving IRT, ensure all routine immunisations are up to date. <\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">Inactivated vaccines are recommended for close contacts to prevent natural infection transmission. Live-attenuated vaccines in contacts are <\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">generally saf<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">e<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">.\u00a0<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">Household infants should still receive the\u202f<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">oral rotavirus vaccine<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">, but because the vaccine virus can be\u202f<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">shed in stool for up to about 1 to 2 weeks<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">, close contacts of patients with\u00a0<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">an IEI<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">\u00a0should practise\u202f<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">strict hand hygiene after nappy changes<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">. The risk of transmission is\u202f<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">low<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">, but these precautions help minimise potential exposure<\/span><span class=\"NormalTextRun SCXW235922310 BCX0\">.<\/span><\/span><\/p>\n<h2><b><span data-contrast=\"auto\">\u5e38\u89c1\u95ee\u9898<\/span><\/b><\/h2>\n<p>\n<\/div><div id=\"mpc_accordion-146a3bed19e445c\" class=\"mpc-accordion mpc-init mpc-transition accordion-tab\" ><ul class=\"mpc-accordion__content\"><li class=\"mpc-accordion__item\"><div class=\"mpc-accordion-item__heading mpc-transition\"><div data-id=\"mpc_icon-706a3bed19e4445\" class=\"mpc-icon mpc-init mpc-transition mpc-effect-fade mpc-icon-hover\"><div class=\"mpc-icon-wrap\"><i class=\"mpc-icon-part mpc-regular mpc-transition  eti eti_arrow_right-down\"><\/i><i class=\"mpc-icon-part mpc-hover mpc-transition  eti eti_arrow_left-up\"><\/i><\/div><\/div><h3>What if a vaccine is inadvertently given to a person receiving IRT?<\/h3><\/div><div class=\"mpc-accordion-item__content mpc-container\" ><div class=\"mpc-accordion-item__wrapper\"><div class=\"uncode_text_column\" ><p><strong>For inactivated vaccines:<\/strong><\/p>\n<p>The individual\/family should be informed of the error using the <a href=\"https:\/\/mvec.mcri.edu.au\/references\/open-disclosure\/\">open disclosure<\/a> framework\u202fand reassured that this is not a safety concern. The inadvertently administered dose is an invalid dose and will need to be repeated (after the appropriate interval has passed) if the patient ceases IRT in the future.<\/p>\n<p>It should also be noted in the patient records that this is considered an invalid dose and must be repeated if the patient ceases IRT in the future. All vaccines should be reported on the <a href=\"https:\/\/mvec.mcri.edu.au\/references\/australian-immunisation-register\/\">Australian Immunisation Record (AIR)<\/a>, whether they were intended for administration or not.<\/p>\n<p><strong>For live-attenuated vaccines:<\/strong><\/p>\n<p>There is the potential for safety concerns if a live-attenuated vaccine is administered to a person with an IEI (regardless of whether they are receiving IRT or not). Immediate consultation with the individual\u2019s treating immunologist and\/or infectious disease physician is required so appropriate management can be commenced (e.g. anti-viral therapy, monitoring).<\/p>\n<p>The individual\/family should be informed of the error using the <a href=\"https:\/\/mvec.mcri.edu.au\/references\/open-disclosure\/\">open disclosure<\/a> framework. The error must also be <a href=\"https:\/\/mvec.mcri.edu.au\/references\/adverse-events-reporting-australia\/\">reported to the relevant authority<\/a> to ensure appropriate follow up and support can be provided.<\/p>\n<\/div><\/div><\/div><\/li><li class=\"mpc-accordion__item\"><div class=\"mpc-accordion-item__heading mpc-transition\"><div data-id=\"mpc_icon-706a3bed19e4445\" class=\"mpc-icon mpc-init mpc-transition mpc-effect-fade mpc-icon-hover\"><div class=\"mpc-icon-wrap\"><i class=\"mpc-icon-part mpc-regular mpc-transition  eti eti_arrow_right-down\"><\/i><i class=\"mpc-icon-part mpc-hover mpc-transition  eti eti_arrow_left-up\"><\/i><\/div><\/div><h3>What if I am not sure if my patient is receiving immunoglobulin therapy for replacement or another indication?<\/h3><\/div><div class=\"mpc-accordion-item__content mpc-container\" ><div class=\"mpc-accordion-item__wrapper\"><div class=\"uncode_text_column\" ><p><span data-contrast=\"auto\">Immunoglobulin replacement therapy (IRT) is typically administered as an ongoing treatment either every four weeks via intravenous infusion (IVIG) or weekly via subcutaneous infusion (SCIG). The doses used for replacement are\u00a0generally lower\u00a0than those used when normal human immunoglobulin is prescribed for its immunomodulatory effects. Typical dosing for replacement therapy is approximately 0.4 g\/kg every four weeks for IVIG or 0.1 g\/kg weekly for SCIG.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">In Australia, the clinical use of immunoglobulin is strictly regulated by the\u202fNational Blood Authority\u202f(NBA), which sets specific eligibility criteria for funded treatment. For patients with an inborn error of immunity (IEI) receiving IRT, the NBA\u00a0maintains\u00a0a defined list of approved conditions that meet these criteria, and the diagnosis must be confirmed by a clinical immunologist.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">As a result, patients receiving immunoglobulin replacement therapy are typically under the care of an immunologist and receive regular infusions (monthly for IVIG or weekly for SCIG) as part of long-term management.<\/span><\/p>\n<\/div><\/div><\/div><\/li><li class=\"mpc-accordion__item\"><div class=\"mpc-accordion-item__heading mpc-transition\"><div data-id=\"mpc_icon-706a3bed19e4445\" class=\"mpc-icon mpc-init mpc-transition mpc-effect-fade mpc-icon-hover\"><div class=\"mpc-icon-wrap\"><i class=\"mpc-icon-part mpc-regular mpc-transition  eti eti_arrow_right-down\"><\/i><i class=\"mpc-icon-part mpc-hover mpc-transition  eti
