BCX0\">treatment <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">option<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\"> for <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">an data-contrast=\"none\"><span class=\"NormalTextRun CommentHighlightPipeRest SCXW13646666 BCX0\">. <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">Rejection of a donated organ <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">is a significant and ongoing concern for transplant recipients<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">.<\/span> <span class=\"NormalTextRun SCXW13646666 BCX0\">Rejection <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">occur<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">s when <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">the recipient\u2019s i<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">m<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">mune system <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">identifies<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\"> the transplanted organ as foreign<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">,<\/span> <span class=\"NormalTextRun SCXW13646666 BCX0\">so<\/span> <span class=\"NormalTextRun SCXW13646666 BCX0\">trie<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">s<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\"> to destroy i<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">t<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">. <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">To <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">av<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">oid rejection, transplant recipients <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">remain<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\"> on lifelong immunosuppressive <\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">medication<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\">,<\/span><span class=\"NormalTextRun SCXW13646666 BCX0\"> leaving them <\/span><a href=\"https:\/\/mvec.mcri.edu.au\/references\/immunosuppression-and-vaccines\/\"><span class=\"NormalTextRun CommentStart CommentHighlightPipeHovered CommentHighlightHovered SCXW13646666 BCX0\">immunocompromised<\/span><\/a><span class=\"NormalTextRun CommentHighlightPipeHovered SCXW13646666 BCX0\">.<\/span><\/span><\/p>\n<p><span class=\"TextRun SCXW152239609 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"none\"><span class=\"NormalTextRun SCXW152239609 BCX0\">Vaccination is particularly important for those who are immunocompromised, due to the increased risk of developing severe disease (which can lead to <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">hospitalisation<\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">, intensive care <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">admission<\/span><span class=\"NormalTextRun SCXW152239609 BCX0\"> or death) if exposed to vaccine-preventable diseases. In people who are immunocompromised, protection <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">gained <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">from<\/span><span class=\"NormalTextRun SCXW152239609 BCX0\"> a<\/span><span class=\"NormalTextRun SCXW152239609 BCX0\"> vaccine<\/span> <span class=\"NormalTextRun SCXW152239609 BCX0\">can be suboptimal as the body is not as easily able to <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">mount a <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">respon<\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">se<\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">. <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">There can <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">also <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">be a loss of protection gained from <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">past <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">vaccines <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">or infections. <\/span><span class=\"NormalTextRun SCXW152239609 BCX0\">Conversely, some vaccines (<\/span><a href=\"https:\/\/mvec.mcri.edu.au\/references\/vaccine-platforms\/\"><span class=\"NormalTextRun CommentStart CommentHighlightPipeClicked CommentHighlightClicked SCXW152239609 BCX0\">live-attenuated vaccines<\/span><\/a><span class=\"NormalTextRun CommentHighlightPipeClicked SCXW152239609 BCX0\">) may be contraindicated due to the potential risk of vaccine-related disease.<\/span><\/span><\/p>\n<h2><b><span data-contrast=\"none\">Recommendations<\/span><\/b><\/h2>\n<h3><b><span data-contrast=\"none\">Serology and screening<\/span><\/b><\/h3>\n<p><span data-contrast=\"none\">With the exception of <a href=\"https:\/\/mvec.mcri.edu.au\/references\/hepatitis-b\/\">hepatitis B<\/a>, MVEC does <a href=\"https:\/\/immunisationhandbook.health.gov.au\/contents\/catch-up-vaccination#laboratory-testing-to-guide-catchup-vaccination\" target=\"_blank\" rel=\"noopener\">not routinely<\/a> recommend serological testing for vaccine-preventable diseases <\/span>as a measure of seroconversion. Instead, children &lt; 18 years of age requiring a SOT are recommended additional doses of vaccines to ensure adequate protection.<\/p>\n<p><span data-contrast=\"none\">As part of transplant work-up, <a href=\"https:\/\/mvec.mcri.edu.au\/references\/bcg-vaccine\/\">tuberculosis<\/a> <a href=\"https:\/\/www.rch.org.au\/immigranthealth\/clinical\/Tuberculosis_screening\/\" target=\"_blank\" rel=\"noopener\">screening<\/a> is recommended for children identified as at-risk for infection (latent or active). Children aged &lt; 5 years should undergo tuberculin skin testing (TST), children <span class=\"TextRun SCXW265982966 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"none\"><span class=\"NormalTextRun SCXW265982966 BCX0\">\u2265<\/span><\/span> 5 years of age should have Interferon Gamma Release Assay (IGRA) performed. The identified risk factors include:<\/span><\/p>\n<ul>\n<li><span data-contrast=\"none\">previous overseas travel to countries with high prevalence of TB<\/span><\/li>\n<li><span data-contrast=\"none\">known contact with tuberculosis<\/span><\/li>\n<li><span data-contrast=\"none\">being born in, or having a close family member\/household contact born in, a TB-endemic country<\/span><\/li>\n<li><span data-contrast=\"none\">children living in remote Aboriginal or Torres Strait Islander communities.<\/span><\/li>\n<\/ul>\n<h3><b><span data-contrast=\"none\">Vaccination timing and schedule<\/span><\/b><\/h3>\n<p><span data-contrast=\"none\">To overcome the challenges with mounting an effective immune response, it is recommended that SOT recipients receive <\/span><span data-contrast=\"none\"><a href=\"https:\/\/www.health.gov.au\/resources\/publications\/national-immunisation-program-schedule?language=en\" target=\"_blank\" rel=\"noopener\">National Immunisation Program (NIP<\/a><\/span><a href=\"https:\/\/www.health.gov.au\/resources\/publications\/national-immunisation-program-schedule?language=en\"><span data-contrast=\"none\">)<\/span><\/a><span data-contrast=\"none\"> vaccines <strong>and<\/strong> additional vaccines <\/span><span data-contrast=\"none\"><strong>well before<\/strong> transplant. <\/span><span data-contrast=\"none\">Planning for this should occur at diagnosis and\/or consideration of transplant. Where a child is overdue routine vaccines, they should be <a href=\"https:\/\/mvec.mcri.edu.au\/references\/catch-up-immunisations\/\">caught up<\/a> as soon as possible. Any live-attenuated vaccines <\/span><b><span data-contrast=\"none\">must <\/span><\/b><span data-contrast=\"none\">be administered a minimum of 4 weeks prior to transplant (these will be contraindicated once immune-suppressed post-transplant).\u00a0<\/span><\/p>\n<p>The following paediatric guidance has been prepared by vaccine experts, immunologists, paediatricians, infections disease physicians and nurse immunisation specialists from MVEC, the Royal Children\u2019s Hospital and Monash Health.<\/p>\n<p><strong>Table 1: Pre-solid organ transplant recipient (SOTR) paediatric immunisation guideline (September 2025)<\/strong><\/p>\n<\/div>\n<p>\n<\/div>\n\t<div class=\"wpb_raw_code wpb_raw_html \" >\n\t\t<div class=\"wpb_wrapper\">\n\t\t\t<style type=\"text\/css\" id=\"supsystic-table-149_79911-css\">#supsystic-table-149_wrapper table { border-collapse: collapse;
