SCXW62371699 BCX0\">accines <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">during a <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">pregna<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">ncy <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">is<\/span> <span class=\"NormalTextRun SCXW62371699 BCX0\">an important way to provide <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">much needed <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">protect<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">ion <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">at a time <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">when <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">a person is<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\"> more vulnerable to <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">severe <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">disease<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\"> and complications<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">. <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">Vaccination during pregnancy also ha<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">s<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\"> the added benefit of <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">allowing the <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">transfer<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\"> of maternal<\/span> <span class=\"NormalTextRun SCXW62371699 BCX0\">antibodies across the placenta <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">to the unborn baby <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">which will <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">provide<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\"> short-term<\/span> <span class=\"NormalTextRun SCXW62371699 BCX0\">protection (<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">passive <\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">immunity<\/span><span class=\"NormalTextRun SCXW62371699 BCX0\">)<\/span> <span class=\"NormalTextRun SCXW62371699 BCX0\">in the first few months of life.<\/span><\/span><\/p>\n<p><span class=\"NormalTextRun SCXW259255171 BCX0\">During pregnancy<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">,<\/span> <span class=\"NormalTextRun SCXW259255171 BCX0\">m<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">any <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">anatomical and physical <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">changes <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">occur t<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">o support <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">the growth of <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">a <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">baby<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">. <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">Some of these changes <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">include<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\"> an <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">increas<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">e in <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">blood volume, <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">increas<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">ed<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\"> demand on the heart <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">and lungs<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">, <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">increas<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">ed<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\"> physical pressure <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">placed <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">on <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">other <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">organs<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\"> due to an enlarged uterus, <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">and changes <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">to <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">the immune system to ensure that the developing baby is not recognised as \u201cforeign<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">\u201d.<\/span> <span class=\"NormalTextRun SCXW259255171 BCX0\">With these changes comes an<\/span> <span class=\"NormalTextRun CommentStart SCXW259255171 BCX0\">increase<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">d<\/span> <span class=\"NormalTextRun SCXW259255171 BCX0\">risk <\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">of<\/span> <span class=\"NormalTextRun SCXW259255171 BCX0\">experiencing severe symptoms and complications if exposed to infections, in particular respiratory infections<\/span><span class=\"NormalTextRun SCXW259255171 BCX0\">.<\/span><\/p>\n<p><span class=\"NormalTextRun SCXW205141903 BCX0\">The<\/span> <span class=\"NormalTextRun SCXW205141903 BCX0\">administration of <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">vaccines <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">during pregnancy<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\"> was historically avoided<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">. <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">However, as <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">technologies <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">ha<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">ve <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">advanced<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">,<\/span> <span class=\"NormalTextRun SCXW205141903 BCX0\">the benefits that some vaccines can offer when adm<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">i<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">nistered during p<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">re<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">gnancy<\/span> <span class=\"NormalTextRun SCXW205141903 BCX0\">now <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">far outweigh any <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">theoretical <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">concerns for safety<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">.<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\"> This has been par<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">t<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">icularly <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">evident<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\"> during pandemics such as <\/span><a href=\"https:\/\/www.who.int\/emergencies\/situations\/influenza-a-(h1n1)-outbreak\" target=\"_blank\" rel=\"noopener\"><span class=\"NormalTextRun CommentStart CommentHighlightPipeRest CommentHighlightRest SCXW205141903 BCX0\">H1N1<\/span><\/a> <span class=\"NormalTextRun SCXW205141903 BCX0\">and <\/span><a href=\"https:\/\/mvec.mcri.edu.au\/references\/covid-19\/\"><span class=\"NormalTextRun CommentStart CommentHighlightPipeRest CommentHighlightRest SCXW205141903 BCX0\">COVI<span class=\"TextRun SCXW132218590 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun CommentStart CommentHighlightPipeRest CommentHighlightRest SCXW132218590 BCX0\">D-1<\/span><\/span>9<\/span><\/a> <span class=\"NormalTextRun SCXW205141903 BCX0\">where pregnant people <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">were more likely to experience <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">severe disease,<\/span> <span class=\"NormalTextRun SCXW205141903 BCX0\">hospitalisation<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\"> and intensive care<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">, <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">affecting <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">maternal and <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">infant <\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">outcomes<\/span><span class=\"NormalTextRun SCXW205141903 BCX0\">.<\/span><\/p>\n<h2>Recommendations<\/h2>\n<p><span data-contrast=\"auto\">A thorough assessment of a person\u2019s immunisation history and administration of any outstanding vaccines prior to pregnancy is an important part of preconception care. Administration of <a href=\"https:\/\/mvec.mcri.edu.au\/references\/vaccine-platforms\/\">liv<span class=\"TextRun SCXW41569501 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun CommentStart CommentHighlightPipeClicked CommentHighlightClicked SCXW41569501 BCX0\">e<\/span><\/span><span class=\"TextRun SCXW41569501 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun CommentHighlightClicked SCXW41569501 BCX0\">\u2011<\/span><\/span><span class=\"TextRun SCXW41569501 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun CommentHighlightClicked SCXW41569501 BCX0\">a<\/span><\/span>ttenuated vaccines<\/a> (e.g. <a href=\"https:\/\/mvec.mcri.edu.au\/references\/measles\/\">MMR<\/a>\/<a href=\"https:\/\/mvec.mcri.edu.au\/references\/varicella\/\">V<\/a>) during pregnancy is contraindicated due to the theoretical risks associated with vaccine-related disease and its impact on a developing infant (see \u2018Contraindicated vaccines&#8217; below for more information). <span class=\"TextRun SCXW41569501 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW41569501 BCX0\">Live<\/span><\/span><span class=\"TextRun SCXW41569501 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW41569501 BCX0\">\u2011<\/span><\/span><span class=\"TextRun SCXW41569501 BCX0\" lang=\"EN-AU\" xml:lang=\"EN-AU\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW41569501 BCX0\">attenuated <\/span><span class=\"NormalTextRun SCXW41569501 BCX0\">vaccines should be <\/span><span class=\"NormalTextRun SCXW41569501 BCX0\">given <\/span><span class=\"NormalTextRun SCXW41569501 BCX0\">a minimum of 28 days prior to <\/span><span class=\"NormalTextRun SCXW41569501 BCX0\">conception<\/span><span class=\"NormalTextRun SCXW41569501 BCX0\">,<\/span> <span class=\"NormalTextRun SCXW41569501 BCX0\">or<\/span><span class=\"NormalTextRun SCXW41569501 BCX0\"> must be withheld until after delivery.<\/span><\/span><\/span><\/p>\n<p><span data-contrast=\"auto\">During pregnancy, there are multiple <a href=\"https:\/\/mvec.mcri.edu.au\/references\/vaccine-platforms\/\">inactivated vaccines<\/a> recommended and funded on the <a href=\"https:\/\/www.health.gov.au\/resources\/publications\/national-immunisation-program-schedule?language=en\" target=\"_blank\" rel=\"noopener\">National Immunisation Program (NIP)<\/a>. Other inactivated vaccines can be administered in special circumstances (e.g. medical risk factors, travel) but are not routinely recommended. Refer to the <\/span><a href=\"https:\/\/immunisationhandbook.health.gov.au\/resources\/handbook-tables\/table-vaccines-that-are-not-routinely-recommended-in-pregnancy-inactivated\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">Australian Immunisation Handbook: Vaccines that are not routinely recommended in pregnancy<\/span><\/a> for further information.<\/p>\n<p><span data-contrast=\"auto\">Correctly timing the administration of vaccines during a pregnancy is important and varies with each vaccine. Guidance on timing is based on whether the resulting immune response is intended to protect the mother, the infant, or both.<\/span><\/p>\n<h3>Influenza<\/h3>\n<p><span data-contrast=\"auto\"><a href=\"https:\/\/mvec.mcri.edu.au\/references\/influenza-vaccine-recommendations\/\">Influenza<\/a> vaccination during pregnancy is safe and strongly recommended to protect the mother from infection and avoid complications of disease. During pregnancy there is an increased effort required by the lungs. Influenza viruses affect lung tissue meaning infection adds further strain. As a result, pregnant people with influenza infection have a higher likelihood of requiring hospitalisation and intensive care than non<\/span><span data-contrast=\"auto\">\u2011<\/span><span data-contrast=\"auto\">pregnant people with influenza infection. Influenza infections during pregnancy also increase the risk of <a href=\"https:\/\/mvec.mcri.edu.au\/references\/preterm-infant-immunisation\/\">preterm birth, babies being born at low birthweight<\/a> and stillbirths.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Antibodies derived from maternal vaccination can also cross the placenta providing some protection to the infant. It is estimated that vaccination during pregnancy can reduce the risk of influenza infection by half in infants less than 6 months of age. This is important because influenza vaccines cannot be administered to infants until they are 6 months of age.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Influenza vaccines are recommended annually and can be given at any time during a pregnancy. If a person is vaccinated before becoming pregnant, they should receive another dose when they are pregnant (minimum 4 weeks between doses). When a pregnancy crosses over influenza seasons, pregnant people are recommended to receive two vaccines, one for each season.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Refer to <\/span><a href=\"https:\/\/mvec.mcri.edu.au\/references\/influenza-vaccine-recommendations\/\"><span data-contrast=\"none\">MVEC: Influenza<\/span><\/a><span data-contrast=\"auto\"> for up<\/span><span data-contrast=\"auto\">\u2011<\/span><span data-contrast=\"auto\">to<\/span><span data-contrast=\"auto\">\u2011<\/span><span data-contrast=\"auto\">date information on current influenza vaccines.<\/span><\/p>\n<h3>Pertussis<\/h3>\n<p><span data-contrast=\"auto\"><a href=\"https:\/\/mvec.mcri.edu.au\/references\/pertussis-whooping-cough\/\">Pertussis<\/a> vaccination during pregnancy is safe and effective and is recommended to protect both the pregnant person and, most importantly, the infant.<\/span><\/p>\n<p><span data-contrast=\"auto\">Infants under 6 months of age infected with pertussis have the highest rates of hospitalisation and death compared with people infected in other age groups. Symptoms include periods of intense coughing (paroxysmal coughing) with apnoea where infants stop breathing for a short period. There can be vomiting and difficulty feeding due to prolonged paroxysmal coughing episodes. Complications of pertussis in infants include pneumonia (lung infection), seizures and encephalitis (brain inflammation) which can be fatal.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Infants require a 3<\/span><span data-contrast=\"auto\">\u2011<\/span><span data-contrast=\"auto\">dose course of pertussis vaccines (doses given at 6 weeks, 4 months and 6 months) to be considered fully protected. Vaccination during pregnancy can reduce the likelihood of infections occurring in infants who are too young to have completed this course by 90%.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">A single dose of vaccine (which is provided in combination with <a href=\"https:\/\/mvec.mcri.edu.au\/references\/diphtheria\/\">diphtheria<\/a> and <a href=\"https:\/\/mvec.mcri.edu.au\/references\/tetanus-prone-wounds-management\/\">tetanus<\/a>) should be administered between 20 and 32 weeks of gestation, in every pregnancy (including pregnancies that are closely spaced). If the pregnant person was vaccinated prior to pregnancy, they should still receive another vaccine within the pregnancy to ensure the infant will be passively protected. If an infant is born within 2 weeks of their mother&#8217;s vaccine, the passive immunity passed onto the infant may be suboptimal.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">Refer to <\/span><a href=\"https:\/\/mvec.mcri.edu.au\/references\/pertussis-whooping-cough\/\"><span data-contrast=\"none\">MVEC<\/span><\/a><span data-contrast=\"none\"><a href=\"https:\/\/mvec.mcri.edu.au\/references\/pertussis-whooping-cough\/\">: Pertussis<\/a> for further information.<\/span><\/p>\n<h3>Respiratory syncytial virus (RSV)<\/h3>\n<p><span data-contrast=\"auto\">Vaccination against <a href=\"https:\/\/mvec.mcri.edu.au\/references\/respiratory-syncytial-virus-rsv\/\">RSV<\/a> during pregnancy is recommended to provide passive immunity to infants. From 2025, vaccination during pregnancy will be funded on the NIP.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">Current data shows that almost all children will be infected with RSV by the time they turn 2 years old. <\/span><span data-contrast=\"auto\">Whilst most infections are mild, bronchiolitis (inflammation of the small airways) and pneumonia (lung infection) can occur and may lead to hospitalisation for supportive measures such as oxygen therapy and rehydration. I<\/span><span data-contrast=\"none\">nfants with <a href=\"https:\/\/immunisationhandbook.health.gov.au\/resources\/tables\/list-conditions-associated-with-increased-risk-of-severe-rsv-disease-in-infants-and-young-children\" target=\"_blank\" rel=\"noopener\">specified risk factors<\/a> such as preterm birth, cardiac conditions and chronic lung disease are more likely to experience severe disease. Clinical trials demonstrate that when vaccines are administered during pregnancy the risks of hospitalisation due to severe RSV infection is reduced by 57% in infants less than 6 months of age. <\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">A single dose of <\/span><b><span data-contrast=\"auto\">Abrysvo <\/span><\/b><span data-contrast=\"auto\">should be administered between 28 and <\/span><span data-contrast=\"auto\">36 weeks\u2019 gestation (NB: Arexvy is <\/span><b><span data-contrast=\"auto\">not <\/span><\/b><span data-contrast=\"auto\">registered for use in pregnancy). There is currently no recommendation for further doses in subsequent pregnancies; however, this may change as more data becomes available. When a pregnant person is vaccinated within 2 weeks of delivery, infant protection is likely to be reduced (many jurisdictions are funding infant RSV prevention programs for infants with medical risk factors and those with suboptimal passive protection).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">For more information refer to <a href=\"https:\/\/mvec.mcri.edu.au\/references\/respiratory-syncytial-virus-rsv\/\">MVEC: RSV<\/a>.<\/span><\/p>\n<h2>Contraindicated vaccines<\/h2>\n<p>The RSV vaccine Arexvy <span data-contrast=\"auto\">must <\/span><b><span data-contrast=\"auto\">not <\/span><\/b><span data-contrast=\"auto\">be administered to pregnant women due to a lack of safety and efficacy data.
